The practice is registered with the ICO (The Information Commissioners Office) for GDPR (General Data Protection Regulation).
To reserve copies of your medical records, you will be asked to complete a Subject Access Request (SAR), please contact reception for further information. The practice operates a GDPR policy which is available on request from reception.
In certain circumstance charges may apply.
All Members of staff at the Practice have received training in all aspects of confidentiality, and have signed a confidentiality agreement. Please ask if you wish to see a copy.
All Individual information is treated in strictest confidence. This may range from the most sensitive diagnosis to the fact that someone has attended the Practice, or is registered with the practice. No information will be disclosed to any non NHS agency, without the permission of the Individual, or your legal representative.
Information about the General Practioners and the practice required for disclosure under this act can be made available to the public. All requests for such information should be made to the practice manager.
